How to tailor the Service Provider's feedback page

By default your Service Provider's feedback page will look a little bit like this (unless the Feedback Centre support team have set it up for you):

Dont worry, this can be tailored by yourselves and you are completely controllable.  To get to these settings click the following in the Wordpress dashboard:

Once you've clicked 'Feedback Centre Settings' you will be taken to a page full of all the controllable elements of the Feedback Centre. Scroll down to the panel that is called "Service Page Layout" which looks a bit like this:

There are two layout options, they are 'Full Width' and 'Two Columns (sidebar)'  select which one you'd like using the dropdown: 

If you select 'Two Columns' you'll notice two extra fields will appear, one depicting the locations of the sidebar and the other allowing you to select the content. If you'd like the sidebar to be on the right, select 'right' and if you'd like the sidebar to be on the left, select 'left'.

To add the content to the sidebar click 'Add Row' and some options will appear:
